About this event
Michelle Sanbria, the Educational Support Specialist from Prime Care will be sharing information on OPWDD (Office for People With Developmental Disabilities). OPWDD is responsible for coordinating services for New Yorkers with developmental disabilities. Once you acquire services with OPWDD, you need to have care coordination through one of two organziations, one of those is Prime Care Coordination. Prime Care serves 8,900 members throughout 41 New York Counties. They support individuals with intellectual and/or developmental disabilities, many of whom have multiple and complex needs, by developing individualized care plans focused on the individual’s personal goals and dreams. Michelle will talk about OPWDD eligibility and how to get services with both of these organizations. Registration is required. This is a virtual presentation on Zoom. The Zoom link will be provided when you register. If you need any reasonable accommodations, please contact Jenny Paxson at jennifer.paxson@libraryweb.org or call 585-872-7075 x6144 at least 7 business days before the event.
