LifeMoves Outreach Team case managers provide assistance to individuals and families experiencing homelessness in San Mateo County. Services case mangers can connect individuals to include: Medical Services: Primary care, dental, vision, mental health and health insurance. Connecting with mobile clinic. Transportation to and from appointments. Food & Meals: Connect with agencies that provide meals and support with food. Housing Support: Information on housing resources, waitlist and shelter. Inpatient/Outpatient: Connect to inpatient drug and alcohol treatment. Outpatient treatment for drug and alcohol. Finding AA/NA/GA meeting locations. Connecting to methadone clinics. Medical and Entitlement Benefits: Assistance with applying for health insurance , Social Security Income and Disability, General Assistance. Pet & Service Animal Support: Assistance with veterinary care, resources for emotional support/service animal certification, and access to items needed for daily care. Additional Support: Advocacy, Reconnecting with family and friends, DMV Identification Vouchers, Food and hygiene kits Transportation to services such as DMV, Human Services, Veterans, Association etc., Employment Services LifeMoves is here to help connect community members with the care and support they need.
