Josephine Ortiz of the SA Food Bank Benefits Assistance Team will be on-site to help you through the process of screening, applying or renewing your benefits for SNAP, CHIP, WIC, Medicaid and more! For a full list of available programs Ms. Ortiz can assist with, please visit the link provided. Walk-ins welcome, appointments preferred. Documents to bring: - Social security cards, ID or birth certificates for all household members - Proof of Income: last 4 paystubs, Social Security Award Letter, etc. - Rent receipt, lease agreement or mortgage payment - Most recent bills for telephone, electricity, gas, water, insurance premium, medical bills and prescriptions - Current bank statements, life insurance policies, real estate tax statements, etc. - Immunization records: TANF and children's Medicaid only Other; Adults
